Dear Parents and Guardians,
With summer upon us, now is a popular time for children to get their annual checkups, if they are due for one, or to have their sports/school/camp physicals and paperwork completed. Life Savers Emergency Room is able to see children of all ages for a full physical exam with a Physician Attestation Form Certifying Fitness to Participate if needed. Keep in mind that even if your child is not due for a physical exam now, we will be offering this service all year round.
Every physical exam includes:
- A thorough review of your child’s medical records and family history
- Check of vital signs, height, and weight
- Vision screening
- Examination of muscles, bones, and joints
- Medication authorization as needed
No insurance is required for the physical, and payment is $25 cash-only.
Take advantage of our Sports Physical Day s to ensure your child is ready for the upcoming school year and sports activities.
- August 17th at Summerwood from 9am-5pm
- August 18th at Willowbrook from 1pm-7pm
